Overview

This continuing medical education program will review how echocardiographic imaging is utilized in the diagnosis and management of patients with hypertrophic cardiomyopathy (HCM). The expert faculty, Dr. Matthew Martinez, MD and Margaret Kurnides, ACS, RDCS, FASE, will discuss how to exclusively target the interests and educational needs of echocardiographers/cardiac sonographers who image patients with a suspicion of, or confirmed HCM.  This program will focus on the critical components of the echocardiographic evaluation as recommended in the 2024 AHA/ACC/AMSSM/HRS/PACES/SCMR Guideline for the Management of Hypertrophic Cardiomyopathy. The faculty will demonstrate techniques and tips aimed at ensuring the objectives of echocardiographic imaging in HCM are met. Margaret will demonstrate optimal image acquisition, and Dr. Martinez will explain the clinical importance of each metric. The faculty will conclude by providing their own insights and responding to learner questions.

Learning Objectives

1

A clear understanding of the structural manifestations of HCM, how they appear on echocardiography, and how progression or improvement of the disease is best tracked by imaging

2

Operational confidence to obtain key images and measurements in the echocardiographic assessment of potential and known HCM

3

A protocolized approach to obtaining the images and measures required in patient follow-up per the required REMS program for the currently approved CMI

Course Agenda

I. Structural Pathology and Pathophysiology of HCM and Visualization of Pathology with Echocardiography

II. Echocardiographic Techniques and Tips to Optimize:

    • Establishment of the diagnosis of HCM
    • Determination of the hypertrophy pattern and measurement of LV wall thickness
    • Assessment for the presence of LV apical hypertrophy and apical aneurysms
    • Assessment of LV systolic and diastolic function
  • Assessment of mitral valve structure, motion, and function, including detection of systolic anterior motion (SAM) of the valve
  • Assessment for diastolic dysfunction with parameters such as E/A ratio, deceleration time of E wave, tissue Doppler imaging (E’ wave), and LA size
  • Assessment of Global Longitudinal Strain (GLS) with speckle-tracking echocardiography
  • Assessment of LVEF and presence and severity of LVOTO, determining peak LVOT gradient at rest and during provocation (e.g., Valsalva maneuver, exercise); use color Doppler and continuous-wave Doppler as needed
    • Assessment for abnormalities in the aortic root and ascending aorta
    • Ultrasound Enhancing Agents

III. When is TEE Needed?

IV. Quick Hits:

    • The REMS Program for Mavacamten: Imaging Requirements (Focus on Systolic Dysfunction)
    • CMR and Multimodal Imaging in HCM: How Other Modalities Supplement Echocardiuography
    • Is There a Role for Artificial Intelligence in HCM Imaging?
