Overview

L’hypertension artérielle demeure l’une des pathologies les plus prévalentes et les plus pourvoyeuses de complications prises en charge en médecine de premier recours. Toutefois, une proportion importante de patients ne parvient pas à obtenir un contrôle tensionnel adéquat malgré l’utilisation d’une plurithérapie antihypertensive. L’excès d’aldostérone — qu’il s’agisse d’un hyperaldostéronisme primaire avéré ou d’une physiopathologie plus large médiée par l’aldostérone — est désormais reconnu comme un facteur contribuant à une hypertension artérielle difficile à contrôler beaucoup plus fréquent qu’on ne le pensait à ce jour. Les inhibiteurs de l’aldostérone synthase (IAS) représentent une nouvelle approche mécanistique ciblant directement la production d’aldostérone par l’inhibition de la CYP11B2 dans la zone glomérulée du cortex surrénalien; ils offrent ainsi une stratégie physiopathologiquement rationnelle, distincte de l’antagonisme des récepteurs des minéralocorticoïdes. Dans ce podcast de formation médicale continue, nous passons en revue les nouvelles données issues des essais cliniques sur les IAS, en examinant leur efficacité, leur profil de sécurité et la façon dont ces agents pourraient s’intégrer dans les algorithmes thérapeutiques fondés sur des preuves pour les patients ayant une hypertension artérielle difficile à contrôler. Les cliniciens disposeront ainsi d’un cadre plus clair pour identifier les patients éligibles à cette nouvelle classe médicamenteuse dans le paysage en constante évolution de la prise en charge de l’hypertension artérielle.
